Holyoke Health Center (HCC)

Our partnership with the Holyoke Health Center began in 2008 when we developed the Mejor Vista, Mejor Vida demonstration project, funded by a 3-year grant from the Jessie B. Cox Charitable Trust. Diabetes is the leading cause of blindness among U.S. adults, but many diabetes patients do not receive regular eye care or have access to vision rehab services. The project integrated MABVI’s expertise in low vision care into HHC’s award-winning diabetes self-management program. MABVI’s Holyoke Low Vision Center is embedded within the Health Center and we continue to collaborate to make sure that patients with diabetes and low vision can successfully manage their health care.
