Job Responsibilities

Responsible for providing direct nursing care to, and monitoring the health care of, individual clients living in agency residences.
Occasional coverage for the RN at agency Day Habilitation Program
Responsible medication administration and oversight medication systems within the residence operating under the MAP program.
Assess and identify the Individual’s needs and functional capabilities including the ability to manage their personal care services independently.
Assist with the coordination of routine medical care as well as hospital admissions and discharges.
Accompany clients to medical appointments and necessary follow up.
Work with client’s interdisciplinary team to achieve optimal health status and independent functioning in the community.
The job is split between two sites, one in Belmont and one in Newton.

Requirements

Must possess a Massachusetts Nursing license as well as experience working with disabled population.
A valid driver’s license and your own means of transportation is required.
Must be currently certified in CPR.
