Job Responsibilities
- Responsible for providing direct nursing care to, and monitoring the health care of, individual clients living in agency residences.
- Occasional coverage for the RN at agency Day Habilitation Program
- Responsible medication administration and oversight medication systems within the residence operating under the MAP program.
- Assess and identify the Individual’s needs and functional capabilities including the ability to manage their personal care services independently.
- Assist with the coordination of routine medical care as well as hospital admissions and discharges.
- Accompany clients to medical appointments and necessary follow up.
- Work with client’s interdisciplinary team to achieve optimal health status and independent functioning in the community.
- The job is split between two sites, one in Belmont and one in Newton.
Requirements
- Must possess a Massachusetts Nursing license as well as experience working with disabled population.
- A valid driver’s license and your own means of transportation is required.
- Must be currently certified in CPR.
